- Two forms of valid photo ID×2CAC or military ID plus a REAL ID-compliant driver's license. Keep both on your person during travel and in-processing.
- Copies of your orders×5Bring the original plus at least four copies. You will surrender copies at multiple in-processing stations.
- DA Form 31 (leave form)×2Signed and dated for your travel and report date.
- ERB / ORB or DA Form 2-1×2Printed within 30 days of your report date.
- Medical recordsHand carry your complete medical record. OCS will not chase your losing unit for it.
- Dental recordsHand carry with your medical record.
- SF 507 physical (National Guard & Reserve)Must be dated within two years of your projected graduation date. Active component candidates do not need this.
- Immunization / shot recordDD Form 2766C or equivalent printout from MEDPROS.
- Most recent AFT / DA Form 705 scorecard×2You will take a record fitness test early in the course. Bring proof of your last one.
- DD Form 93 and SGLV 8286Record of Emergency Data and SGLI election. Update both before you leave your unit.
- Security clearance documentationProof of a current or in-progress SECRET clearance. A clearance is a graduation requirement.
- College transcripts / degree conferral letterSealed official transcript showing your conferred baccalaureate degree.
- OCS selection memorandum / commissioning packetYour board results and any approved waivers.
- Marriage certificate, dependent birth certificates & SSN cardsOnly if you need DEERS updates. Bring originals or certified copies — the ID lab will not accept photocopies.
- Direct deposit informationVoided check or a bank letter with your routing and account number.
- Power of attorneyStrongly recommended if married or you carry financial obligations. Legal Assistance on post can draft one, but having it in hand saves weeks.
- Paper copy of emergency contactsNames, phone numbers, and addresses on paper. Your phone will be secured for long stretches.
- Driver's license, registration & proof of insuranceOnly if driving to Fort Benning. Required to register a POV on post.
- Travel itinerary and receiptsLodging, tolls, and flight receipts for your travel voucher.
- Prescription list, profiles & waiversA written list of every medication with dosages, plus any approved medical or religious accommodation memos.
